Your Right to a Good Faith Estimate
Under the No Surprises Act
If you are uninsured or choosing not to use your insurance for services at Sway Health, you are entitled to receive a Good Faith Estimate (GFE) of expected charges before your scheduled services.
What Is a Good Faith Estimate?
A Good Faith Estimate outlines the estimated cost of services that are reasonably expected for your mental health care based on the information we know at the time. This includes services provided by your Sway Health clinician through our telehealth platform or companion app, Baseline by Sway.
Please note:
The estimate does not include unexpected costs that could arise due to complications, emergencies, or other changes in your care.
If your actual charges are $400 or more above the Good Faith Estimate per clinician, you have the right to dispute the bill.
What to Do If You’re Billed Over the Estimate
If you receive a bill that is $400 or more over the Good Faith Estimate, you have options:
Contact Sway Health
Let us know about the discrepancy
Ask for the bill to be updated to match the estimate
Request a payment plan, financial assistance, or to negotiate the charge
Phone: (833) 688-2274
Email: support@swayhealth.co

How to Start a Formal Dispute
You can also begin a dispute resolution process with the U.S. Department of Health and Human Services (HHS) if:
Your bill is $400+ more than the GFE, and
You start the dispute process within 120 calendar days of the original bill date.
There is a $25 fee to initiate the process. If the dispute reviewer agrees with you, you’ll only pay the GFE amount. If the reviewer agrees with Sway Health, you may have to pay the higher bill.
